Why eat low phosphorus?

Phosphorus restriction in chronic kidney disease (CKD) stages 4-5 helps prevent secondary hyperparathyroidism, bone disease, and vascular calcification. Typical targets fall between 800-1,000 mg/day, but the exact number depends on lab values, phosphate binders, and dialysis status. As with potassium, the target should be set with a renal team.

Naturally-occurring phosphorus in whole foods (meat, dairy, legumes, whole grains) is absorbed at 40-60%. Phosphate additives in processed foods, sometimes hidden behind names like "monocalcium phosphate", "sodium phosphate", "phosphoric acid", or simply "PHOS" on a label, are absorbed at over 90%. Avoiding inorganic phosphate additives is often a bigger lever than restricting whole-food phosphorus.
